I will add a little drawing-lesson ( from the beginning ), to show how
to merge different Images and Text to a new one.
In the meantime, You can download the needed Tool ( Freeware )

Download Pixelformer :
http://www.qualibyte.com/

1. Image - merge :

We open PIXELFORMER twice, 1. the loaded Image and 2. a working-area
Using < IMPORT > from File-menu we resize the Original using < PROPERTIES > to the needed Size

Image

Inside the working-area, we resize the loaded Image we want to merge with the Original

Image

The merged Image we move to the needed Position.
In case the Result ( proportion ) is to small or to big, we delete the merged Image
and moving back to the Working-area to do the needed Changes.

Image

We EXPORT the Result. Don't forget to activate < Premultipled alpha > !!!

2. Text - merge :

I understand.

I think using a Screencapture-program with included Ballon / message-painting, will do the Job.
I have tested some.
I will include a Download-Link ( Freeware )
